BOOKING AN APPOINTMENT

NEW PATIENT FORMS

As a new patient, you need to fill out a patient record and tell us about your dental problems, previous treatments, medications you currently take, and general health conditions. We want to know everything about your health to pick the correct treatment and minimize the risk of complications.

INSURANCE

To complete the patient form, we kindly ask you to provide the details of your insurance policy or membership plan.

AT THE OFFICE.

After arriving at The Art of Smiles, our receptionist will greet you with a warm smile and help prepare for your appointment.

When Dr. Hernandez is ready to see you, an assistant will take you to the office to receive a consultation and treatment.

EXAM, X-RAY, HEALTH HISTORY

Talk to Dr. Hernandez about your issues, expectations, and health history to make sure you are on the same page. After a careful visual examination, you might need to go through X-ray diagnosing to detect conditions invisible to the naked eye.

TREATMENT PLAN

As soon as Dr. Hernandez sees the whole picture of your oral health, she will come up with a treatment plan and alternatives. You will be given full information about their efficiency, duration, and cost.

FAQ

What is the difference between general and cosmetic dentistry?

General dentistry focuses on maintaining or restoring your oral hygiene through prevention, diagnosis and treating oral diseases. Cosmetic treatments focus solely on the appearance of your teeth and smile. Please visit our Services page for a list of our most popular treatments.

What will it cost?

Since all patients are different, your initial comprehensive exam must be completed before establishing your treatment plan and the fee for care. The fee for dental treatment can vary considerably depending on the type of problems and the complexity and length of treatment. An approximate fee can usually be determined at the initial visit; but on occasion, some initial treatment or further diagnostics must be completed before the final treatment plan can be established. Our philosophy of practice is to treat as conservatively as possible to attain treatment goals.
